Mar Zutra challenged Rava’s prohibition of manipulating with a stone for concern of hair-removal, by exclaiming “should he endanger himself?”; if (presumably) it is not pikuach nefesh then why would this be grounds for heter?

It is an intuitive argument. It just can’t be that every Shabbos one must endanger himself, even if it does not necessarily mean death.
